Puppy Sales Contract with Health Guarantee

The following agreement is between Regina Pollard (breeder), hereafter referred to as "Seller," and ____________________________________________, hereafter referred to as "Buyer."

In consideration of a purchase price of $______________, the Seller transfers all rights, privileges, and responsibilities associated with the ownership of this puppy, of the ________________ breed, to the Buyer, as of the date and time specified below, with exception to the following:

Buyer agrees to spay/neuter puppy upon it reaching six months of age or shortly thereafter, but no later than eight months of age. ______________ (Buyer initials)

Buyer will provide copy of spay/neuter paperwork or be prepared to pay $200 penalty for breaking this contract, or be taken to court for not honoring this contract. _____________(Buyer initials)

Buyer agrees to notify Seller of any health conditions issues that develop with the dog after purchase. ______________ (Buyer initials)

Seller guarantees the health of this puppy at the time of sale. If puppy is shipped, a veterinarian will have examined this puppy within ten days of sale and issued a health certificate. It is recommended that Buyer take puppy to their veterinarian within 72 hours of delivery for a get-acquainted visit and checkup. If a health problem is discovered during this visit, a full refund will be provided upon return of the puppy. There are no known cogenital conditions known to the breeder at this time. By first birthday, if the pup becomes sick and/or dies due to a birth defect, or health neglect on the Seller’s part, and the genetic condition is sufficiently serious in the opinion of two qualified veterinarians, at least one of whom is selected by the seller, to warrant euthanization of the dog, the Seller agrees to refund the full purchase price paid by the new owner. This guarantee is void if death or illness is due to neglect on the part of the Buyer. This guarantee is good until pup reaches its first birthday.

The following steps, paperwork included, have been taken to ensure you have purchased a healthy, happy puppy:

1st shot & 1st worming given at 6 weeks of age: _________________

4th puppy shot & 4th worming at 16 weeks of age: ________________

Rabies shot given at 16 weeks of age: __________________________

New research strongly recommends fourth puppy shot at 16 weeks (4 months) to ensure protection against parvovirus, distemper, etc.!! It is so much cheaper to vaccinate than to treat the puppy for the virus it has picked up.

By signing below, both parties have read, understand, and agree with the terms of this contract.
